This application collects name, phone number, email address, age, sex and photograph for organizational membership administration.
Information is used to create and administer membership records and generate an organizational ID card. Phone and email are not printed on the member-facing card.
Records are stored in a private database and photographs in private object storage. Administrative access is restricted to authorized administrators.
Registration requires affirmative consent. The system records the consent version and timestamp.
The organization operating this service should define its retention period and process for correction/deletion requests and publish the appropriate contact details.
Important: This is an organizational membership/identity card, not a government-issued identity document.
